A Summary Care Record is an electronic record that can help give you safer, faster care by giving halthcare staff instant access to important information about your medicines, allergies and medicines that make you ill.

A Summary Care Record will allow hospital staff etc see your medication and allergies instantly, this is especially useful in an emergency. They will not be able to see what you've talked to the Doctor about, or past/current medical problems you may have. Only your Medication & Allergies. Only NHS staff will have this access.
If you do not wish healthcare staff to be able to see your medication and allergies, you can opt out. Click here to download the form and hand to reception, or ask at reception for one.
